Math Exit slips are designed to bring closure to your math lessons. Each slip has fewer than four tasks to complete and should take students approximately 5 minutes to finish.

I found these exit slips very useful for forming my small groups - it was quick, easy, and all I had to do is make copies. I reduced them on the copier so that I could save paper and the kids really liked the small slips - it was novel for them. The slips aligned with my lessons quite well!!
