Suggestions for use:

Begin by giving the assessments that you think will be most useful. Analyze this data to determine where you should begin with the student(s) and create an intervention plan, using the Progress Monitoring worksheet.

Use the intervention practice sheets that are included 3-5 times per week with student (according to the plan you created). I suggest laminating these practice sheets and then you can have students use a dry erase or vis a vis marker to complete their practice. This way, the sheets are re-useable. Place them in a binder and you are ready to go anytime you need to complete an intervention activity. Keep track of your daily work with this student on the Intervention recording sheet.

Before beginning the intervention work, choose 1-2 of the assessments that you will use for WEEKLY progress monitoring. Keep track of their weekly progress on the Progress Monitoring worksheet.

I suggest an six-week cycle for each intervention. After six weeks, decide if you will continue, move on to something new with the student, or discontinue their intervention work.

These materials will help you immensely in your PLC discussions about your struggling students, and will serve as valuable evidence of your interventions in case you need to recommend Tier 3 support.
