This math mindset questionnaire asks students to identify how they feel about math. Do students believe that anyone can be successful in math? How do students prefer to work in math? What do students do when faced with a challenge in math? All of this information can be useful to determine the attitudes that students have towards math and to give you a good starting point when working with them.
The first section, "Agree or Disagree", uses the visuals of a thumbs up and thumbs down to respond to various prompts. This should make it easier for ELs as well as students who struggle to to answer these questions. The second section, "Finish the Sentence", just has 3 brief prompts for students to write about their math experiences.
