Communicate with students and parents about missing assignments using these simple slips! Simply write the missing assignments and the due date, and require student and parent signatures.

There are two versions - 4 cards per page, and long slips that are 3 per page. I typically use the cards when communicating fewer assignments over a short period of time, and staple them to an assignment book or another assignment. The slips are great when we near the end of the marking period/unit if students are missing more assignments.

This resource also includes a form for you to track missing work. Whenever I fill out a slip for a student, I track it on my form as record of communication and to follow up about whether the assignment was submitted.
