As an Instructional Coach, I model MANY lessons for the teachers in my building. Every time I model a lesson, the observing teacher fills out this reflection form. We then review the form as part of our coaching debrief meeting.
I highly recommend this form to help teachers stay engaged while watching the model lesson and to reflect on how to apply with they have observed.
