This is a modification tracker spreadsheet I made to help me as an in-class-support teacher in a 7th grade ELA classroom.

You can add your own student names on top and then tally each time a student needs a modification. This helps me understand which modification are helping students and what other modifications could possibly help my students.

You can also change the modification, but there are 39 common modifications there already.
