This is a one page worksheet (more in depth than sample provided) that can be placed in a student's monitorred reading (independent reading) folder. It provides more in-depth Common Core aligned documentation that they are reading critically when reading independently.

Objectives addressed: author's tone, setting, conflict, theme, text to self connections, etc.

Note: Have the students cut the top 3 boxes off of the page and paperclip each box to the page they will stop at. This is used as a reminder bookmark to go back to the form.
