This is a note I use in my classroom for when students have to sit in time out in music. I send this home to communicate with the parents. There are check boxes that go with my rules posters that are in my music room (Make good choices, Use kind words, show respect, involve yourself, care for equipment). There is also a place to write comments about what happened. It has a place for you to type in your own contact info if a parent wanted to contact you after reading the note.
