This year my district adopted the My Math Program as our new math curriculum. The chapter assessments are completed on the computer, which led me to want something that I can physically send home with student scores. I created these quarter sheet notes that makes it easy to print a class set and record the students' names and data.
If a student didn't pass an assessment, I will print out the actual computer test for parents to view and sign, but for the majority of the class, these chapter assessment notes have been sufficient in notifying families.
These notes aren't just great for families, they also help students become aware of their learning. I pass out the test notes (or any test for that matter), and students record the data into their binders before they take the tests home to their families.
