This google form assignment is a great addition to a nervous system unit. It can be used as extra practice to reinforce what you've covered if you cover neural plasticity with your nervous system unit. If not, it's still great background information that will help your students have greater depth of knowledge about the nervous system. It is great as homework, sub plans, or used for independent work during class.

The google form is divided into two sections. Section one is an introduction to neural plasticity and section two covers the types of neural plasticity. Each section includes 5 questions with a mixture of multiple choice and true/false for a total of 10 questions for the assignment.
