The following is a very useful format to communicate with parents. I have already used it with my students. We all know that as teachers, we don't have time to write notes home on a constant daily basis. Therefore, this is just a fill out form that will allow you to communicate with parents on student's behavior and responsibilities. It's just a half page note for you to print, keep some copies on your desk and use it as a rule is broken by a student in or out of your classroom.
