I use this to help hold students accountable when they are not turning in their work on time.

Directions for students:

· Staple this pass to your late assignment.

· I allow two OOPS passes per semester.

· I reserve the right to NOT accept the OOPS passes on certain assignments or projects.

Students also are asked to explain why the assignment should be accepted late. File is 100% editable using Microsoft Word (file is .rtf).
