This practice log is for the teacher who'd rather collect data digitally than by hand first. Students practice along with whatever you'd like them to throughout the week and then at the end of it, complete this practice log that can be copied and edited to fit your needs.

Questions are based on Levels of Understanding scale using images within the form to define each level.

Simply duplicate the image box as many times as needed (I like to put it above each question that uses it so students aren't all over the form looking for it).

I then make a copy for each week that matches what I taught so students are self-assessing the skills we did in class that they practiced at home.
