Description
A checklist to track your students' mastery of the PA Common Core Math Standards. One form for each standard is included. (I combined OA and G if you wanted to use less paper) Includes all four math domains; Numbers and Operations, Algebraic Concepts, Geometry, and Measurement and Data for 5th grade. One form is editable so you can type in each student's name. The other form you can just print as is.
I write in the names on the left, date goes before the standard and the standard number goes after. I did not include the standard number because sometimes I teach them out of order.
This is set up so you can easily see the data per child for each standard for a pre assessment, mid assessment and a post assessment.

Standards

CCSS5.MD.A.1
Convert among different-sized standard measurement units within a given measurement system (e.g., convert 5 cm to 0.05 m), and use these conversions in solving multi-step, real world problems.
CCSS5.MD.B.2
Make a line plot to display a data set of measurements in fractions of a unit (1/2, 1/4, 1/8). Use operations on fractions for this grade to solve problems involving information presented in line plots. For example, given different measurements of liquid in identical beakers, find the amount of liquid each beaker would contain if the total amount in all the beakers were redistributed equally.
CCSS5.MD.C.3
Recognize volume as an attribute of solid figures and understand concepts of volume measurement.
