I created this Parent Contact Log Google Form to easily keep track of all the times I reach out to parents. This form is completely editable to fit your purposes to contact parents of your students. I love that after completing the form (each time I contact a parent), I can at anytime click on the responses tab and choose to generate a spreadsheet of the the responses. Voila, there is my documentation, professionally presented and easily interpreted. Also, under the response tab there are automated data charts representing what you entered each time you completed the form.
