Parent Teacher Conference form I have used to help me organize what I want to discuss during parent teacher conferences. The download includes:

**Parent teacher conference form for grades K-5 and 1 blank form. The forms are saved in powerpoint so that you can insert text boxes for your own comments and goals. You can also print the forms and hand write them but I prefer to use text boxes. I focus a lot of social emotional learning in my room so this conference form has a box for SEL goals.

**Parent teacher conference tips sheet and sample academic and SEL goals.

**An example of a conference form I filled out for a student in my room this year.

**I plan to add to this document as I develop more material for Parent Teacher conferences.

This is perfect for parent-teacher conferences. I love that it comes in a power point so it is easy to edit. I also really loved having something for parents to work on for the end of the year. Thank you!

I think this is such a great form, and I plan on using something like this for my conferences this year. The two things I would improve on this are: 1) More flexibility in editing - I would pay more for this! I'd like a more professional looking form and would like to take out the clip art, if I wanted. Or make the boxes bigger for more information. 2) The ability to type into the form. This would be faster and help with with filling out next year's forms for kids who may have similar goals. Or pasting in learning targets, etc. Thank you for this resource, and I look forward to seeing more from you!

Hi! Thank you so much for the feedback. Based on what you suggested I added a plain, clip art free slide with bigger boxes for text. To type into the form, just add a text box when it opens in powerpoint and you can type right onto the slide. I hope this helps! Thanks so much for downloading this product and for your feedback.
