This document is intended for supervisors or administrators who conduct informal visits and want a simple form that allows for quick note taking. It is also structured to encourage the supervisor to provide positive feedback to the educator being observed. It is quite generalized and so provides a tremendous amount of flexibility. Can be used for observations in all subjects at all grade levels. This document can serve in traditional classrooms and can also be used for assessments of presenters in other types of settings.
