If you are looking for a quick printable resource to help you track specific negative behaviors in your self-contained classroom or special education program, this resource is for you!
These daily data sheets are designed to be a simple way for you to track behaviors that may be occurring.
These frequency data sheets are for one student, and for one day only. If you have students with a high occurrence of behaviors, these may be helpful documentation for you prior to starting a Behavior Plan or referring for evaluations or FBA.
Target behaviors being documented on these frequency data sheets are physical aggression and self-injurious behaviors (SIB).
To use:
- Decide what time intervals you wish to track and write in under the time column. I typically choose 15 minutes.
- Write down what your scheduled activity is for each time slot.
- Track the behaviors with tally marks.
- Also note any positives that are happening!
Includes:
- Blank option (you write in times, activities)
- AM Class Option (times included 8:00-11:00 with 15 minute intervals)
- PM Class Option (times included 12:00-3:00 with 15 minute intervals)
- Blank option (you write in times, activities, and what behaviors you are tracking)
You will receive a PDF download with 4 printable data sheets (not editable in this format).
