Progress Monitoring Data Collection Spreadsheet/Graphs

Description

As a special education teacher, progress monitoring and writing progress reports are one of my least favorite parts of the job. I've spent years trying to streamline the process of collecting and recording data to make it as easy as possible. With these data sheets, not only is my data easy to record, but everything is in one place and easily visible. With one click, I can pull up my data to share with parents, students, and administrators. At the end of the reporting period, averages are automatically calculated, graphs are automatically created, and I can print and go! I hope you find this spreadsheet as helpful and I've found it.

THIS RESOURCE IS FOR SCHOOLS THAT HAVE TRIMESTERS!

What's included:

A google spreadsheet with the following tabs:

-Data collection sheet: Spots to record data for various goals, including reading comprehension, reading accuracy, reading fluency, math computation, math problem solving, percentage based math goals (up to 3, but you could easily copy and paste to add more), and written expression for 5 domains (again, you can copy and paste if you measure more than that). Written expression can be monitored based on a rubric or based on percentages.

-Baseline spreadsheets: At the beginning of the year, I enter the student's baseline for each of their IEP goals. That way, as the trimester progresses and I collect data, I can see how much they are growing or if additional intervention is needed. The averages from data entered in the previous spreadsheet automatically populates on this form for easy visibility. There is also a second baseline spreadsheet to update once you hold the student's annual IEP meeting.

-Auto-created graphs for each goal you monitor. My district requires me to send graphs home with IEP progress reports. I used to spend hours creating these at the end of each trimester, but with this spreadsheet, they create themselves! 19 graphs included. Just print the spreadsheet (be sure to choose print workbook), and everything is done with just a few clicks of the mouse.

I no longer dread progress report time and I feel better prepared to provide needed interventions throughout the year with up-to-date and easily visible data!
