Basic psychoeducational report template. Includes table frames for grades, English proficiency assessments (state-specific to CA), and state/district assessments to be reported, as well as headers for other needed information (observations, background, etc.). Processing areas are included, but not exhaustive and coincide with Dehn's model as opposed to CHC. Score explanations were compiled and represent score ranges that averaged based on most commonly used assessments. They do not follow the publisher guidelines for all individual assessments. Please consider this and your district's/organization's requirements when reporting scores. However, the frame can be modified and duplicated (copied) to be used in representing individual assessment score explanations if that is your preference/requirement. The interpretive table also separates the average band into multiple threads (low average, average, high average) in an effort to capture the major differences among the various publisher standards across assessments. Additionally, eligibility criterion charts for the most frequent categories are included at the end but are based on state-specific eligibility criteria (Barclays Official California Code of Regulations) and are not exhaustive. For example, the more low incident categories such as deaf/blind are not included. Please confirm the criterion with your own state's ed codes and regulations. Information may be [bracketed] to indicate the need for you to sub in student or test-specific information; information may also be highlighted or both. There are no frames for academic tests or previous assessments. This was done to avoid copyright infringement. The headers, however, are present.
