This may be used as a formative assessment as it was designed or you can use it as practice. I can also be used as a large group discussion. That is why I left it in WORD.doc form (so you can change what you need to serve your purpose).
This assessment asks students the following questions:
a. What is the domain and range of the function?
b. Does this function have a maximum or a minimum?
How do you know?
c. What is the maximum or minimum value?
d. Can a quadratic function have both a max and a min value?
Explain why or why not.
e. Determine any zero(s) by looking at the graph.
