Fluency is an important skill to instill in all readers. This form allows the students to practice their fluency on their own. It has been designed to be used with any book.
The student will first record themself reading the passage. The student will then listen to the recording, while completing the form. The form asks the students to reflect on their volume, rate, pronunciation, and any words that they found to be difficult.
There is also a bottom portion for them to complete next time. At a later time, the students will complete a second recording of the same passage and compare how they have improved since the last time. They will also write about what they will plan to do in the future.
In my classroom, I complete this assignment using the camera video app on the iPad. Typically, we read a passage at the beginning of the month and again at the end of the month. In the time between, I review their reflection and tell them whether I agree or disagree, and list any additional techniques that they could try (if necessary).

I usually have my students read a passage to the iPad (phone, computer, anything that records!) at the beginning of the week. The students then listen/watch the recording and complete the form. We then practice reading that same passage throughout the week. On Friday, they record themselves again, listen, and complete the rest of the form.
