This home reading incentive program includes 15 reading passports with different clip art. Choose a different passport for each month of the school year!
The Reading Passport includes:
• Assembly Directions
• Instructions for Use
• 15 Reading Passports
• 18 Bookmarks
• 3 Certificates: 50 Days of Reading, 100 Days of Reading, Super Reader
• Prize Ballots
All student pages are ink-friendly and, although this product contains some seasonal clip art, it is not tied to any religious holidays. Each half-page passport includes the correct number of circles to correspond with the number of days in the month.
Print back-to-back, cut in half, and fold. Then, stamp or initial the passport each time your students read at home. The prize ballots can be used as an additional incentive for students who have met their reading goals for the month.

Love the idea, and I think the kids will like it, too. Do you have the parents sign and then reward a sticker in the passport? I'm unclear on how you know the students completed their reading at home.
It’s up to you! Some years I have given out the stamp or sticker after the student re-reads their home reading book to me in class. Other years, they get a stamp or sticker after they bring their book back to class. It’s used it in place of a traditional reading log because most patents don’t like filling them out.
Plan to use this with my GT class next year as extension.
