There are 3 different versions of reflection form for you so you should find one that works for your elementary, middle, or high school students. (I have multiple printed and use the one most appropriate for the individual child.)

Reflection forms are the second to last step on my ladder of support (notice, not a ladder of discipline) posted in my classroom. This is NOT a punishment. Please make sure to watch the guide video. If after multiple whole class reminders, a couple of individual redirections (not public) and an optional two minute break, if the student is still not meeting expectations, I'll ask them to take a moment in my chill zone to fill out my reflection form.

8/9 times out of 10 it gives me a much needed window into HOW they're feeling and WHY they're feeling that way. It is important, to circle back at some point in the day (it may not always be at the end of class) to check in with the student about what they wrote. As I said, so often I learn that they didn't eat that morning, or they're angry with someone at home and it gives me insight to their behavior or lack of participation.
