Each nine weeks you can meet with students in academic conferences to create goals for the next nine weeks OR use the forms in small groups to create an accountability system. Copies can be shared with teachers and/or parents to keep them informed about current goals.

My students use this so that they can participate in their conferences. If they don't attend their conference, I give it to parents so they can see what their learner thinks and feels about their learning. At this age, I feel it is essential for students to have a voice and be accountable for their learning.
