This handout is perfect for having your students reflect on their performance each time they receive a report card. I assign this handout as a homework assignment for students to complete with their parents when they receive their report card each quarter. Then, I hold onto these handouts, so I can be mindful of the goals my students have set for themselves in the coming quarter.
