This interactive activity involves parents, students and the teacher. After a student receives their report card, give this form to your students to fill out along with their parents. Everyone participates in the process and parent and student reflect on what they observe. What can they (parents, students and teachers) improve on? What did they do well? This form helps students and parents reflect what is working or not. What can be done to improve or to affirm what is working. Parents, teachers and students all collaborate.
