Description

Editable in MSWord and Google Docs. If you are a special education teacher or speech therapist (or any case manager writing present levels on the IEP) where your students are in general education for most of the day, these editable teacher feedback forms will make your life easier. In addition, one version is also available on Google Forms!

This resource is a fully editable Microsoft Word document so you can personalize it with your name, and also change/add any information you need.

In addition, the MSWord document you download can also be opened/edited with Google Slides: Go to NEW in Google Drive and choose "upload file." After you choose this file, click on it in the bottom box to open it... Then choose "Open in Google Docs." Then it will open, and you can change text, etc. Remember to rename it.

Form 1: This form will help you find out how the student is currently doing (i.e. "present levels of performance"). Keep printed copies of the form handy, so you can grab one two weeks before any initial, annual, or triennial Individual Education Plan (IEP) meeting. Simply fill out the staff member's name, student's name and grade, type of meeting, the date you are putting it in their box, and the day it is due back to you. You could also use it with other special ed staff if you are the person that has to fill out the IEP (i.e. occupational therapists, adaptive PE specialists, autism specialists, etc.)

I chose to set it up so it follows the same order as the Present Level page of the IEP program our district uses. (I'm in California and our district, like hundreds of others, uses "SEIS - Special Education Information System" online IEP program). The end result will be a very professional looking document - which will make you look good in the eyes of principals, teachers, etc.

I know teachers are very busy, so they can mostly check and circle what they need; then add short comments or test scores as needed. My teachers love this form as they can fill it out pretty quickly! I will also fill out or cross off info I already have so teachers know I am saving them even more time.

Form 2: This is a quick "Check In Form" so you can get a snapshot of how your students are doing. Teachers can mark one of two boxes, or write a quick note with a student success or concern. This is a way for you know who you need to follow up with. Two versions provided. Not only can case managers use this, but staff in many areas can use it (occupational therapy, adapted PE, physical therapy, nursing, teachers of visually impaired, orthopedic/physical impairments, deaf/hard of hearing, as well as orientation and mobility specialists, psychologists, etc.) This form is also available as a Google Form. Please read pages 11-12 of the resource to read instructions on how to get it from me as a Google Form.
