Restroom pass using QR codes!

Create a Google Form.
I used two questions on my form:
What subject are we doing? (I did multiple choice answers)
What is your name? (they fill in their own name)

Students fill out the questions before leaving for the restroom. The Google Form will populate into a Google Sheet with the time stamp of the day and time a student left the room to use the restroom.

Once you have the URL for the Google Form, simply paste it into a QR code generator (many websites are available to do this for free) and paste your QR code onto the template!
