Are you starting an elementary school robotics club or tech club? This is a simple application I created to collect information from students. It includes:


1. A description of the club you are offering: What, Where, When

2. A general information form to collect parent contact information, allergies, and t-shirt size.

3. A student answer form where students write why they want to become a club member.

4. A homeroom teacher rating page.


I use this form to make the selection process for our school clubs fair to all who apply.
