This freebie is an editable Google Form for digital collection of SLP therapy data. This form may be used for in-person therapy or tele-therapy. However, because this form is not specific to speech and language therapy, it can likewise be used for data collection by other therapists and staff members such as PTs, OTs, and Intervention Specialists.

The following content is included in this Google Form (though, all content is editable to meet your individual needs):

• Student information
• Date
• Attendance
• Type of session
• Direct therapy time
• Goal(s) or objective(s) addressed
• Quantitative data collected
• Qualitative data collected
• Level of prompting provided
• Planning for future sessions
• Other notes

(Additionally, a nice bonus to using digital data collection is that you may make as many copies of the form as you need for your caseload and then download information into Google Sheets to keep track of the data if desired.)

This file was created in Google Forms under Google Drive. To learn more about how to add this file to your own drive, click here. Essentially, TPT will automatically make a copy of the form, make a TPT folder in your drive, and add the folder to your Google Drive.
