This student-friendly form can be used with students to keep track of their STAR assessment progress monitoring. You may use this in student data notebooks and data chats.

This form uses the achievement levels (FAST levels 1-5). If you need the benchmark levels, use this link.

Please note this is intended to be used with STAR Renaissance assessments. The file includes a tracking sheet for early literacy, reading, and math. Each tracking sheet includes data for FAST equivalency scaled score, grade equivalency, achievement level measurements, and reflection questions for 3 progress monitoring assessments (PM1, PM2, PM3). After each assessment, students will write their scores and color the box to show achievement level.

The are two versions for each subject: one with the achievement levels in color (red, yellow, blue, light green, dark green) and one with the achievement levels without color.
