I have used these tools in my elementary classroom to make standards-based grading easier for myself, as well as to have documentation for parents to show why their child got the score they did. These forms are for teachers who use a 1-4 scale to grade students, where 3 is considered mastery.
Use the scaffolding plan to map out how students will get from their prerequisite knowledge to mastery of a particular standard.
There is also a form for parents to see what the expectation is for a score of 1, 2, 3, or 4.
