I give this to my students every Monday with their weekly vocabulary words listed on the bottom. They are to complete two of the six menu options throughout the week. This gives them a sense of choice and they are more likely to complete the homework.

This one says 'Science' on it, but if requested, I will gladly make more for other subjects. The activities could easily be used for vocabulary words for any subject.
