This packet is comprised of miscellaneous materials that I’ve created over the years for homework practice; quick, informal assessment, and/or work samples. If you download this packet, please leave feedback. Although the majority of my materials are designed to meet the specific needs of the students on my caseload at the time of development, your feedback will be considered when I develop other materials.


The worksheets in this packet are designed to target the following sentence formulation skills:
 Changing Yes/No Questions to Sentences (3 levels of difficulty)
 Changing Sentences to Questions
 Formulating Sentences given 2 words
 Formulating Sentences given 3 words
 Formulating sentence using verb phrases (all verb tenses)

This resource was perfect for some of my fourth grade students via distance learning. I was able to share my screen with them and type into the document. The prompts were age appropriate and worked well for my students to expand their sentences and increase their complexity.
