Description
This self-assessment worksheet guides students through examining their current reading work -- reading logs, reading worksheets, tests, Reader's Notebook, Reader's Workshop Post-it's, etc. -- in order to ascertain their own personal reading strengths and challenges (weaknesses).
Students are then asked to reflect and set specific reading goals. They are also asked to write-out what they need to do in order to achieve the goals, as well as the supplies or support they need in order to achieve the goal.
This was originally designed to accompany Reader's Workshop's first unit, Launching Reader's Workshop. It is appropriate for third through eighth grade. I use it with my fifth graders and find it very useful! It encourages to students to engage in Reader's Workshop lessons and apply themselves!
