I created this to use as I please. I Googled and searched for a basic behavior chart for elementary students and could not find one, so I decided to create one. I use it to keep data for executive functioning goals. For example: For 5 ten minute increments, Jane Doe will stay on task 3/5 times in order to participate in whole group and independent activities. I also created a chart to record this data. The smiley face chart is to be laminated and given to the student. The recording sheet is for the teacher's own personal data keeping. I will upload that next as Data Chart for Tracking Behavior.
