Description
This form can be used to evaluate the performance and participation of each student in a cooperative learning group. Each group member writes the names of everyone in their group and gives them a rating. The purpose of this particular evaluation is for it to be simple for students to fill out quickly at the end of group time. It also includes a column for the student to evaluate his or her own performance. The evaluation form is made for groups of five students and includes the following criteria:
- Participated in discussion
- Maintained a positive attitude
- Contributed to the work
- Stayed on task
It also includes a basic scale of 1-3 with a signature line where students verify that their ratings are accurate.
