Lead students through a general introduction or review for Slope-Intercept Form. Starts out with the general form of the equation, identifying the slope & y-intercept, and 2 examples of equation where students determine the slope & y-intercept. Then, it goes on to give the steps to write an equation from a graph, and finally steps to draw a graph given an equation. This would be great for partners or small groups to work on as a Do Now, an Exit Ticket, or to be used as a study guide. Answer Key included. Students fill in the blanks as you go along to help them get more out of it than simply giving them all of the information. It is also faster than having them write all of the notes themselves. If you fill in the examples, this could also be used as an assessment.
