These progress monitoring sheets can help with data collection needed for phonics, phonemic awareness, and/or reading instruction.

Each sheet acts as a collection of data for one student across the span of five weeks. As you plan your lessons, pick out words you are already including in your instruction and write them into the appropriate spaces on the progress monitoring sheets. When teaching your lessons and completing activities, record what each student does in response to the given word(s).

I personally use these sheets to hold myself accountable for data collection. Although I am teaching the same lesson to a small group of students, I can tailor the words given to specific students based on their own individual goals and needs.
