This is a one page baseline or data sheet for spatial concepts. There is a section for expressive (labeling) and receptive (identifying) of various spatial concepts. There is also space for the student's name and space for multiple dates.

Targeted Spatial Concepts Include:

Expressive (Label): on, above/over, below/under, in, out, in front, behind, between, and beside/next to/by

Receptive (Identify): on, above, under, by, in front, behind, between, beside, out, over, below, in, next to and off.
