The focus of this test is to determine understanding and proficiency with square roots. This involves accurately simplifying square roots with composite numbers, include variables with exponents. Students should also be able to simplify and combine square roots with addition, subtraction, multiplication and division, including rationalizing the denominator and reducing.

The test is twenty questions long, with plenty of white space. There is a google forms version of this you can choose to access, too.

The test is formatted in Word, which means you can easily change the test from multiple choice to free response. An answer document is included.
