This document was created as an individual student sheet. The teacher can record points the student has earned for each category. The categories included are participation, homework, quiz, bell work, and tests. I have also included a column for the grade and extra credit. You may change the categories to accommodate for your classroom grading and activities. I have also included a place to record absences, restroom passes, and in school suspension days. I designed it so that you can place dates for each 6 weeks or 9 weeks. There is also a comments section for parent contact or other important information needed for the individual student.
