This is quick multi-use formative assessment tool that can be used in any subject. Use this as an entrance or exit ticket when it is time to move on in a unit. Project this onto your projector and have students stick a sticky note to the box that suits them best.

I find doing this anonymously works really well, because students don’t feel as if they’ll be punished for not being ready to move on. Alternatively, you can have students put their name or initials on the back of the stick notes to keep it private from their peers, and use the information for small groups or scaffolding.
