Are you looking for help with behavior issues in the classroom? If so, these student behavior forms are what I have used for years in my middle school classroom and they have been so helpful. When a student is disrupting class or not following our classroom expectations, these forms serve as a way for the student to reflect on their behavior. There are four different versions of the Stop & Think forms, and the questions are editable so you can change the wording if you would like. *MUST HAVE POWERPOINT TO EDIT THIS RESOURCE.

What's Included:

  • Editable PowerPoint file of 4 different versions of the Stop & Think form
  • PDF file of 4 different versions of the Stop & Think form
  • Description of how I use the Stop & Think forms in my classroom

How I Like to Use this Product:

These forms have been extremely helpful in my middle school science classroom! If I have to redirect a student more than once in a class period, I use one of these forms. I also use these forms to document how many times students need to be redirected to share with parents and principals.
