I have included files in the Zip Folder that include PowerPoint, Microsoft Word, and PDF. I included Microsoft Word so you can change the product to meet your needs.
I use this test as a pre and post test. I give it to the students before I start the unit, to help me with my small groups and direct instruction. I use the recording sheet to record my pre and post test scores for each student. If a student does not understand the questions, I put a circle in the recording box. If they do understand, I put a check mark in the box. Those students who have not mastered a common core benchmark, I will reteach again at a later time.
I always place the Common Core standards at the top of each of my assessments. I grade each standard separately and place the score on the line next to the standard. I give this story problem assessment towards the end of the school year. I would have already taught money, measurement, story problems; addition and subtraction with regrouping. It does include one and two step story problems. I included one money problem and one measurement problem too.
Some teachers like to score their problem solving questions in different ways. I left the scoring blank so teachers can use their discretion when grading. I also separated the last page from the first two pages because I feel the open ended response may be optional.
My students track their progress in their Leadership Data Notebooks. They set goals periodically throughout the school year in each academic subject. Once they have mastered their goal, they write a new goal.
