This very handy form can be used by teachers for any student studying to learn violin, viola, cello or bass in a studio, orchestra or group class setting. Parents love it!

If you do not have Excel, you could try opening it using OpenOffice or other shareware application. You may alternatively download the same form as a PDF in the shop!<<
Students also can gain an accurate snapshot of their progress and specific things they need to work on once they receive this report.
The form is in black and white and it contains a rubric for assessing where a student currently is in his or her learning. The focus is on progress and improvement. On the second page, list the score next to the category and then simply total the scores. There is space to enter comments for each student, for each area of their learning or for a general comment. There is a space for the teacher's signature at the bottom. Students love this, especially in combination with a gold sticker.
The form is two pages, and was created in A4 size so you might need to adjust it for 8.5 x 11 inch paper. It could be printed front-to-back if desired.
