School Social Workers have to keep track of a lot of students and its always important to be able to build relationships with students to help with the counseling process. Student Background Assessment Notes are a summary of students main information. These notes were created with the intention of being able to see student information at a glance. The documents can be filled in by adobe or if preferred they can be printed in bulk to input information by hand.
These notes include:
* Name of the student
* Date that you are meeting with student or creating a file for student
* The student's age and grade
* Referral
* Classifications for student case (Behavior, Mental Health, Check In, and Reporting - CPS Cases or Mandatory Reporting Cases)
* Contact Information for student and any special considerations (Custody Conflict, Homeless, Title I Hope, IEP or 504)
* School Schedule and student's schedule
* Academic Counselor Assigned to student
* Health Considerations for student
* Behavior History for student
The labels and listings are based off of Clark County School District's Infinite Campus labels so they match terminology with Clark County School District. The product is a digital download, which can be downloaded from the TPT website for use. The download should be immediately available upon purchasing the item.
