Description
This Student Behavior Observation & Communication Chart is used to document and communicate student behavior in a consistent and objective manner across the school day. It tracks both positive behaviors (such as following directions, participation, respectful communication, and task completion) and areas for growth (such as disruptions, off-task behavior, refusal to comply, or difficulty with transitions).
The purpose of this chart is to provide families with regular, structured feedback on their student’s daily behavior patterns in the classroom. It helps build a clear home-school connection by highlighting strengths as well as identifying behaviors that may require additional support or intervention.
Teachers record observable behaviors at designated intervals (e.g., class periods, blocks, or specific times of day) using consistent criteria. The chart is designed to be adaptable for different grade levels and student needs, including elementary, middle, and high school settings, as well as special education and general education environments.
This tool is not intended to label or define a student, but rather to provide actionable information that supports communication, reflection, and collaborative problem-solving between school and home. Patterns observed through the chart may be used to guide positive behavior supports, interventions, and instructional planning.
