Behavior Point Sheet

This point sheet is used to help students practice positive behaviors and stay successful in class. During each class period, the teacher will rate the behavior using a 0–1–2 point system based on the expectations listed below.

Scoring System:

• 2 points – Met the expectation independently
  
• 1 point – Met the expectation with 1 reminder
  
• 0 points – Did not meet the expectation
  

The goal is for the student to practice:

• Staying in her seat and remaining in the classroom
  
• Completing assigned work
  
• Being respectful to classmates and teachers
  
• Keeping hands and feet to self
  
• Using an appropriate voice and not making unnecessary noises in class
  

Teachers may also provide comments to help reinforce positive behavior or note areas for improvement.
